I don't think a sleeve button is missing . It's the same on both sleeves So I think he did it on purpose OR it's the style of the jacket.   As for the epaulet I think it's just a thread. |

I think he unbuttoned those jacket sleeve buttons to pull through his Shirt sleeves.
